My name is Carolyn, and I am a practicing Functional Nutritional Therapy | Mind Body Practitioner

living in the beautiful seaside city of Newcastle, NSW, Australia.

For every symptom there is a potential imbalance or deficiency which is causing your body stress and burden.

This is what I will be starting to explore with you through the completion of a Nutritional Assessment Questionnaire.

Now this is no ordinary health questionnaire, this goes beyond anything you may have filled out before.

This Assessment tool lives within Nutri-Q, a bespoke nutritional portal that is the brainchild of the Nutritional Therapy Association.

I am so grateful to be qualified to utilise Nutri-Q and especially this very unique assessment.

The Nutritional Assessment Questionnaire (NAQ) offers questions around 320 different symptoms.

No stress at all, just grab a cuppa and allow around 30 min, you deserve to take that time out for your health!

Once you have filled this out and submitted (via the NUTRI-Q portal), I will advise the timeframe required for analysis and

formulation of your personalised recommendations (usually 3-5 working days)

How it works:

​

✨Fill out the below form, providing your name, suburb/country and email address and provide 50% payment. ($44)

​

✨Using this information I will set you up in Nutri-Q and activate you as a client. 

 
✨You will receive an email from Nutri-Q with instructions on creating a log-in. Once logged in, you will be provided with privacy policy and terms before heading to your main client page. Look for your "to do" items and get started!

✨ Once completed, click submit and I will receive the notification. I will then commence my analysis and get back in touch with you via email.

 

What you recieve:

✨ Once payment is finalised in full, you will receive a personalised Symptom Burden Graph, showing where your body may be experiencing the highest symptoms of stress .

 

✨ You will be provided with information around potential conditions and nutritional deficiencies. These are solely based on your answers so make sure you fill these out as honestly as you can.

 

✨Once I have completed my analysis I will formulate and provide 3 tailored nutritional recommendations and 2 lifestyle recommendations, along with supporting handouts to help jump-start your health journey.
